• Statement of Salvation Experience and Commitment to Christian Ministry
• All official college transcripts for all schools attended. A bachelor’s degree is required.
  - One institutional transcript must display evidence of having a conferred baccalaureate degree issued to the applying student
• Church Endorsement from a Pastor or Church Leader
• Two Personal and/or Professional References
